What Forms Do I Need Prior To My Appointment?
You need to sign the Consent Form, fill out the Health History Form and Patient Information Form, and read the Welcome Form prior to your appointment. Please send filled out Consent Form and Health History Form to Kathleen at least 72 hours prior to your tele visit. You can scan forms and send them to [email protected]
